Do you accept insurance?

4 Corners Sports Performance is an out of network provider for physical therapy services. We believe in providing the highest quality, personalized service for all of our clients and we feel insurance is often a barrier to this. Our model allows for patients to be seen for a longer duration and at a lesser frequency than most traditional in-network physical therapy clinics, allowing for a more thorough treatment and a quicker return to full function. Additionally, private pay services allows for the treament of multiple areas of pain or dysfunction in the same visit based on what is the highest priorty for you and your wellness. We are pleased to accept all HSA and flex spending cards for our PT services along with flexible payment options as needed.

Do I need to see my doctor before seeing a PT?

No! In the state of Colorado, you do not need to see a physician before seeing a physical therapist, nor do you need a doctors order for physical therapy. Research supports direct access assessment and treatment from physical therapists to be a viable option for orthopedic and sports related injuries, leading to less pain and faster recovery. When necessary, a doctors referral, or referral for diagnostic imagining including MRI and x-ray may be ordered directly by your physical therapist to help get answers to your problems.

We are located at 6452 Fig St. Suite C Arvada, CO 80004, located adjacent to Move by Morelli, east of Gardenia Street off of 64th Avenue.

Physical Therapy services specialize in treating a variety of orthpedic and sports medicine injuries including muscle strains, shoulder and neck pain, throwing injuries to the shoulder and elbow, low back pain, disc herniations, hip pain, ACL rehabiliation, and post operative care. Sports Performance can help accelerate your rehabiliation and get you back to your sport of choice faster and stronger than before. Our team is well trained in working with athletes young and old who want to return to activity following a variety of complex injuries and can help design a custom plan to get you back feeling better than ever.
